Contact requests
If you request a callback, we collect your preferred name, phone number, preferred callback time, and confirmation that you want us to contact you. We also keep the request time, follow-up status, and a broad referral category, when available. Please do not send medical details, insurance identifiers, or payment information through this form.
Smarter Wellness and its assigned SPAM Agency administrator use the information to respond, arrange a visit, and manage follow-up. A request is not a confirmed appointment and does not enroll you in marketing. Contact fields are encrypted before storage. Email alerts link to the private inbox and do not include your name or phone number.
Your website choices
Optional website measurement is off until you enable it. If enabled, this website counts page loads, phone-link clicks, and the start of a contact request by day and broad source category. Care pages share one category; we do not send the particular service page, search text, form contents, names, phone numbers, or a visitor identifier to the measurement endpoint. Counts describe website actions, not completed calls, appointments, or revenue.
A preference stored in your browser remembers your choice. When enabled, a temporary browser-session value can remember a broad referral source such as Google, ChatGPT, or Claude. No advertising pixels or cross-site visitor IDs are used by the site. We honor Global Privacy Control and Do Not Track by leaving optional measurement off. Accepted callback requests and staff-confirmed outcomes are counted separately to operate the inquiry service.
Hosting and service providers
The website and its private data service are hosted through OpenAI Sites and Cloudflare. Alerts are sent through Amazon Web Services email delivery. Authorized staff sign in through ChatGPT to access the private inbox. The hosting providers process network information to deliver and secure the service and may set security cookies. A short-lived, keyed network hash is used for abuse limits; it is separate from usage counts, contains no saved raw IP address, and expires within an hour.
These providers support website operation. Contact requests are not sold or sent to advertising networks by this site. Links to maps or other external sites have their own privacy practices.
Retention and access
Callback requests expire from the active inbox after 90 days. Expired records are removed during routine inbox or request cleanup. Authorized staff can delete a request earlier. Access and status-change logs are retained for up to 90 days under the same cleanup policy. Aggregate website counts are retained for up to 400 days. Provider backups and operational security logs follow the providers' retention practices.
The private inbox is limited to the practice and its assigned agency administrator. We use HTTPS and restricted access, but no website can promise absolute security.
